Characterization of antibiotic overuse for common infectious disease states at hospital discharge

Objective:
To evaluate rates of and outcomes associated with antibiotic overuse at hospital discharge for patients with common infectious diseases states.
Design:Single-center, respective cohort study.
Setting:A large, academic medical center in the Midwest United States.
Patients:Adult patients who received antibiotics for community-acquired pneumonia (CAP), uncomplicated cystitis, or mild, non-purulent cellulitis. Patients were excluded if they did not receive antibiotic(s) upon hospital discharge, were pregnant, severely immunocompromised, had concomitant infections, died during hospitalization, or were transferred to another hospital or to an intensive care unit.
Methods:Data were abstracted from the electronic medical record of ambulatory antibiotic orders for included patients based on inpatient encounters from August 1, 2021 through July 31, 2022.
Results:Of the 182 patients included in the study, antibiotic overuse was common for all three infectious disease states: CAP (n = 87/125, 69.6%), uncomplicated cystitis (n = 21/28, 75.0%), mild, non-purulent cellulitis (n = 28/29, 96.6%). The prevailing reason for overuse was excessive antibiotic duration (n = 127/182, 69.8%; mean antibiotic duration 5.39 vs. 8.32 days, p = 0.001). Antibiotic overuse was associated with approximately one additional day in the hospital (2.48 vs. 3.32 days, p = 0.001), and an increase in emergency department visits within 30 days after discharge (1 vs. 31, p = 0.001) compared to patients without antibiotic overuse at discharge.
Conclusion:Antibiotic overuse was prevalent upon hospital discharge for these three common infectious disease states. Transitions of care should be prioritized as an area for antimicrobial stewardship intervention.

Introduction:
Traumatic spinal cord injuries (tSCI) are common, often leaving patients irreparably debilitated. Therefore, novel strategies such as nerve transfers (NT) are needed for mitigating secondary SCI damage and improving function. Although different tSCI NT options exist, little is known about the epidemiological and injury-related aspects of this patient population. Here, we report such characteristics to better identify and understand the number and types of tSCI individuals who may benefit from NTs.
Materials and Methods:Two peripheral nerve experts independently evaluated all adult tSCI individuals < 80 years old admitted with cervical tSCI (C1–T1) between 2005 and 2019 with documented tSCI severity using the ASIA Impairment Scale for suitability for NT (nerve donor with MRC strength ≥ 4/5 and recipient ≤ 2/5). Demographic, traumatic injury, and neurological injury variables were collected and analyzed.
Results:A total of 709 tSCI individuals were identified with 224 (32%) who met the selection criteria for participation based on their tSCI level (C1–T1). Of these, 108 (15% of all tSCIs and 48% of all cervical tSCIs) were deemed to be appropriate NT candidates. Due to recovery, 6 NT candidates initially deem appropriate no longer qualified by their last follow-up. Conversely, 19 individuals not initially considered appropriate then become eligible by their last follow-up.
Conclusion:We found that a large proportion of individuals with cervical tSCI could potentially benefit from NTs. To our knowledge, this is the first study to detail the number of tSCI individuals that may qualify for NT from a large prospective database.

Models of insulin secretory vesicles from pancreatic beta cells have been created using the cellPACK suite of tools to research, curate, construct and visualise the current state of knowledge. The model integrates experimental information from proteomics, structural biology, cryoelectron microscopy and X-ray tomography, and is used to generate models of mature and immature vesicles. A new method was developed to generate a confidence score that reconciles inconsistencies between three available proteomes using expert annotations of cellular localisation. The models are used to simulate soft X-ray tomograms, allowing quantification of features that are observed in experimental tomograms, and in turn, allowing interpretation of X-ray tomograms at the molecular level.

Traditional transect survey methods for forest antelopes often underestimate density for common species and do not provide sufficient data for rarer species. The use of camera trapping as a survey tool for medium and large terrestrial mammals has become increasingly common, especially in forest habitats. Here, we applied the distance sampling method to images generated from camera-trap surveys in Dja Faunal Reserve, Cameroon, and used an estimate of the proportion of time animals are active to correct for negative bias in the density estimates from the 24-hour camera-trap survey datasets. We also used multiple covariate distance sampling with body weight as a covariate to estimate detection probabilities and densities of rarer species. These methods provide an effective tool for monitoring the status of individual species or a community of forest antelope species, information urgently needed for conservation planning and action.

The mammal family Tenrecidae (Afrotheria: Afrosoricida) is endemic to Madagascar. Here we present the conservation priorities for the 31 species of tenrec that were assessed or reassessed in 2015–2016 for the IUCN Red List of Threatened Species. Six species (19.4%) were found to be threatened (4 Vulnerable, 2 Endangered) and one species was categorized as Data Deficient. The primary threat to tenrecs is habitat loss, mostly as a result of slash-and-burn agriculture, but some species are also threatened by hunting and incidental capture in fishing traps. In the longer term, climate change is expected to alter tenrec habitats and ranges. However, the lack of data for most tenrecs on population size, ecology and distribution, together with frequent changes in taxonomy (with many cryptic species being discovered based on genetic analyses) and the poorly understood impact of bushmeat hunting on spiny species (Tenrecinae), hinders conservation planning. Priority conservation actions are presented for Madagascar's tenrecs for the first time since 1990 and focus on conserving forest habitat (especially through improved management of protected areas) and filling essential knowledge gaps. Tenrec research, monitoring and conservation should be integrated into broader sustainable development objectives and programmes targeting higher profile species, such as lemurs, if we are to see an improvement in the conservation status of tenrecs in the near future.
Antimicrobial Stewardship in Outpatient Settings: A Systematic Review

Objective
Evaluate the effect of outpatient antimicrobial stewardship programs on prescribing, patient, microbial outcomes, and costs.
DesignSystematic review
MethodsSearch of MEDLINE (2000 through November 2013), Cochrane Library, and reference lists of relevant studies. We included English language studies with patient populations relevant to the United States (eg, infectious conditions, prescription services) evaluating stewardship programs in outpatient settings and reporting outcomes of interest. Data regarding study characteristics and outcomes were extracted and organized by intervention type.
ResultsWe identified 50 studies eligible for inclusion, with most (29 of 50; 58%) reporting on respiratory tract infections, followed by multiple/unspecified infections (17 of 50; 34%). We found medium-strength evidence that stewardship programs incorporating communication skills training and laboratory testing are associated with reductions in antimicrobial use, and low-strength evidence that other stewardship interventions are associated with improved prescribing. Patient-centered outcomes, which were infrequently reported, were not adversely affected. Medication costs were generally lower with stewardship interventions, but overall program costs were rarely reported. No studies reported microbial outcomes, and data regarding outpatient settings other than primary care clinics are limited.
ConclusionsLow- to moderate-strength evidence suggests that antimicrobial stewardship programs in outpatient settings improve antimicrobial prescribing without adversely effecting patient outcomes. Effectiveness depends on program type. Most studies were not designed to measure patient or resistance outcomes. Data regarding sustainability and scalability of interventions are limited.

Constant Versus Rhythmic Muscle Tension in Applied Tension

The research aimed to examine ways in which applied tension for blood-injury phobia may be improved. The effect on blood pressure of two forms of muscle tension under three different rates of breathing is reported. Forty-five participants were randomly assigned to use either constant or rhythmic muscle tension under three different breathing rates (i.e., fast, slow, and normal). The effects on systolic and diastolic blood pressure were measured. Results indicated that both constant and rhythmic forms of muscle tension increased systolic and diastolic blood pressure. However, rhythmic tension was significantly more effective than constant tension at raising diastolic blood pressure. Breathing rate had no differential effects on blood pressure. Suggestions are made about modifications of Applied Tension in the treatment of blood-injury phobia.

A comparative test of the correlated evolution of flightlessness and relative brain size in birds

Secondary flightlessness has evolved independently many times in birds. Morphological changes in the pectoral girdle and flight feathers and changes in body size have been associated with the evolution of flightlessness, and it has also been suggested that flightless birds have relatively small brains. We therefore tested whether flightlessness is related to changes in relative brain size. Relative brain size was compared between volant and flightless species using both conventional statistics and modern comparative methods within nine taxonomic groups. No significant difference was found between flightless and volant species in six of these groups, regardless of whether body mass or tibiotarsal measurements were used as estimates of body size. Species with relatively smaller brains compared with their volant relatives were the great auk Pinguinus impennis, the kakapo Strigops habroptilus and some species of penguin. Thus, we found no evidence of a general correlation between the evolution of secondary flightlessness and the evolution of relatively small brains in birds. This suggests that neural requirements are not significantly different between flightless and volant species, although our methods may have overlooked subtle neurological changes that do not result in markedly different endocranial volumes.
